For Health Professionals

Referral process

Referrals can be made by: 

  • Aged Care Home RN/EN, in consultation with lead nurse 
  • GPs 
  • Victorian Virtual Emergency Department (VVED) 
  • Health service clinicians 

External referrals should be made by using Residential in Reach Referral Form
Internal referrals should be submitted via e-referral on Opal. 

Referral timing guidlines

  • Urgent (Same Day):  Call Residential in Reach and submit the referral form 
  • Within 1-3 Days:  Submit the referral form 
  • Within 7 Days:  Submit the referral form
