The renal pharmacist clinic is linked to the Renal specialty clinic at Ballarat, which is focused on diagnosis and early intervention of chronic kidney disease.

The Hospital Admission Risk Program (HARP) pharmacist aims to reduce emergency department presentations and hospital admissions of HARP clients, by providing timely medication reviews at home.
The HARP pharmacist visits clients at home, with a HARP Care Coordinator. They review medication management and compliance, provide information and education about medications, and liaise with supports including GPs, community pharmacists and the HARP team to ensure optimal patient care in the community.

The Paediatric Outpatient Pharmacist is part of the General Paediatrics Outpatient Service at Grampians Health. Paediatricians refer patients with neurodevelopmental and behavioural conditions to the POP pharmacist for specialist medication reviews.
The POP clinic provides telehealth consults for children and their carers, assessing the effects of new or changes to medications, providing tailored medication education and troubleshooting difficulties experienced at home. The POP clinic liaises with community services (GPs, community pharmacies, social services) to optimise medication management and access to medications.
