Pharmacy Outpatient Clinics

Our Pharmacy team is involved in various outpatient clinics to ensure safe and up-to-date use of medications for patients.

Clinics

Grampians Health runs three cardiology pharmacist outpatient clinics at Ballarat: 

  • rapid access atrial fibrillation 
  • heart failure 
  • interventional cardiology

The clinics are run via telehealth. They involve specialist medication consultation, optimisation and education, and are linked with the relevant cardiology service.  

To access this service:  

  • GP referral: Fax directly to the clinic on 03 5330 3130 
  • Internal referral: Through emergency and acute wards 

The renal pharmacist clinic is linked to the Renal specialty clinic at Ballarat, which is focused on diagnosis and early intervention of chronic kidney disease.  

The renal pharmacist clinic is run via telehealth. It offers specialist medication reviews linked with the kidney care team, and patient education on kidney disease and medicines use. 

To access this service:

  • An appointment with the renal pharmacist clinic will be offered to you as a part of your initial or repeat consultation with the kidney specialists. If you would like to schedule an appointment with the pharmacist, contact 5320 4107 and we will do our best to assist.

The Hospital Admission Risk Program (HARP) pharmacist aims to reduce emergency department presentations and hospital admissions of HARP clients, by providing timely medication reviews at home. 

The HARP pharmacist visits clients at home, with a HARP Care Coordinator. They review medication management and compliance, provide information and education about medications, and liaise with supports including GPs, community pharmacists and the HARP team to ensure optimal patient care in the community.

  • Community clinic/GP referral: Forward patient information (with letterhead) to Central Intake

The Paediatric Outpatient Pharmacist is part of the General Paediatrics Outpatient Service at Grampians Health. Paediatricians refer patients with neurodevelopmental and behavioural conditions to the POP pharmacist for specialist medication reviews.  

The POP clinic provides telehealth consults for children and their carers, assessing the effects of new or changes to medications, providing tailored medication education and troubleshooting difficulties experienced at home. The POP clinic liaises with community services (GPs, community pharmacies, social services) to optimise medication management and access to medications.  

To access this service: 

  • Patients attending the General Paediatrics Outpatient clinics at Ballarat Hospital can be referred by a doctor to the POP clinic using the dedicated referral form  
  • POP clinic appointments are accessed via the hospital telehealth webpage
