Our Older Adult Community Mental Health and Wellbeing Team supports the mental health and emotional wellbeing of people aged 65 years and over. Grounded in the principles of dignity, recovery, and person-centred care, we work in partnership with older adults, families, and carers to promote wellbeing, independence, and connection.

Who we care for

The Older Adult Community Mental Health and Wellbeing stream is part of the Grampians Mental Health and Wellbeing Service (GMHWS) at Grampians Health.   

We are the first point of contact for both the client and their family, in accessing mental health services for an aging person. We provide assessment and treatment for people aged 65 years and older. 

This service is available to eligible people across the Grampians region.

We understand that mental health needs in later life are shaped by unique experiences, transitions, and challenges. Our team offers respectful, trauma-informed care that supports older adults to live meaningful and connected lives. 

We provide treatment, care, and support where it best supports the person—whether at our community mental health site, in the home, or in the broader community.
