Hospital Admission Risk Program (HARP)

The Hospital Admission Risk Program (HARP) provides short-term care coordination for people with complex medical and/or psychosocial issues. 

Who we care for

We support people who use hospitals often or are at risk of being hospitalised and may benefit from support to self-manage their health and remain independent. 

HARP specialises in assisting people with: 

  • Unstable diabetes 
  • Chronic respiratory disease 
  • Chronic heart disease 
  • Complex psychosocial concerns 
  • Complex aged needs (including falls) 

To be eligible you must be 18 years or older.  

Our HARP program services the following areas:

  • Moorabool Shire 
  • Hepburn Shire 
  • Golden Plains Shire 
  • Hindmarsh Shire 
  • Horsham City Rural Shire 
  • Northern Grampians Shire
  • Pyrenees Shire
  • Shire of Ararat
  • West Wimmera Shire 
  • Yarriambiack Shire

  • find ways to improve your health  
  • link you with other services
