Home Support

Our Home Support program delivers a range of services to support your choice to remain living at home safely and with confidence as you get older.  

Who we care for

We provide care and services to those who are eligible for assistance as assessed by My Aged Care. These services are provided through the Australian Government ‘Home Care Packages/Support at Home’ and ‘Commonwealth Home Support Program’ (CHSP). 

What you can expect

The following services are available through Grampians Health.  

Our allied health team provides expert care and health support for people through a range of services.  

There are multiple programs available through Grampians Health to help you maintain your independence and support you to remain at home.  

Entry into these programs is through an assessment and enquiries should be directed through My Aged Care.  

Commonwealth Home Support Program (CHSP) 

CHSP provides entry level co-ordinated services for older people who need assistance with daily tasks as part of living independently at home.  It also provides support to carers through planned respite services. 

CHSP supports people over the age of 65yrs (or 50 years and over for First Nations people).

Home Care Packages 

Home Care Packages are a higher level of coordinated services for older people. It delivers a range of services to support your choice to remain living at home safely and with confidence.  

Our experienced staff partner with you to promote your independence, wellness and an enhanced quality of life. In addition to individual care management, services may include: 

  • home help (such as cleaning, laundry, gardening, meal preparation, home maintenance) 
  • support to keep you connected to your community (such as shopping and transport) 
  • personal care 
  • equipment, including medical supplies 
  • allied health services 
  • home modifications (such as handrails, ramps) 
  • nursing, day care and medical support, if required 

There are currently four levels of packages with associated levels of funding to best support your needs: 

  • Level 1: Basic care needs 
  • Level 2: Low-level care needs 
  • Level 3: Intermediate care needs 
  • Level 4: High level care needs 

Please be aware that as of the 1 November 2025 the Department of Health and Ageing is transitioning to the new Support at Home program. 

Our Continence team provides expert care and health support for people through a range of services.

The Wimmera Dementia Support Group is a free and confidential support network for people who care for someone living with dementia. It provides a safe space for carers who look after someone living with dementia in the community.  

What you can expect  

In a friendly, confidential and supportive setting, carers can:  

  • hear from guest speakers who share helpful advice and information  
  • connect with others who understand what it’s like to be a carer  
  • take a short break from their caring role  

Where to attend meetings  

Meetings are held on the first Monday of every month (except public holidays). 
Time: 10:30am – 12:00pm

Our district nursing team provides expert care and health support for people who require in-home nursing services.

Our Memory Support service offers specialised practical and emotional support to people living with dementia or memory difficulties, and their carers and families.   

Who we care for  

You may be eligible for this service if you are:  

  • aged 65 years and over (50 years and over for Aboriginal and Torres Strait Islander people)  
  • under 65 and diagnosed with or suspected to have younger onset dementia  
  • living at home, either alone or with family/carers  

You will not be eligible if:  

  • memory loss is related to an acquired brain injury  
  • you are receiving a Home Care Package  
  • you are living in permanent Residential Aged Care  

What you can expect  

Living with memory loss or dementia can have a significant impact on daily life – not only for the person affected but also their family and carers.   

Our Memory Support service helps you in managing memory loss and everyday challenges. Support is delivered in your own home. Assistance focuses on the areas that matter most to you. The goal is to help you stay independent and living at home for as long as you choose.  

Our team works closely with other health professionals to make sure you and your carer get the right support at the right time. The nurse can make referrals to other health services if needed to help you stay safe and well at home.  

Access and referrals  

  • Referrals can be made by your GP, other health professionals, family or friends  
  • Self-referrals are also welcome  
  • Referrals must go through My Aged Care

Our Social Support Groups encourage people to be independent and to stay active and connected through social activities.
