Healthy habits for life

The Healthy Habits for Life program (formerly Healthy Weight Management program) is a long-term approach to building healthy habits and managing your weight. It’s designed to give you the tools, knowledge and confidence to take charge of your health.  

Who we care for

This program is for people who: 

  • have significant health conditions, and/or 
  • are living with a higher body weight 

What you can expect

Healthy Habits for Life supports you to set and reach your health goals.  This involves weekly education and exercise sessions, with a focus on client self-management through SMART goal setting (Specific, Measurable, Achievable, Relevant, and Time-bound).  

The 8-week program consists of weekly: 

  • 1 hour group education session – covering nutrition, exercise and the psychology of health behaviours 
  • 1 hour gym session – with our Exercise Physiologist 
  • 30 minutes to set individual goals for the following week 

You’ll have the chance to meet one-on-one with a clinician half-way through and at the end of the 8-week program to ask questions, seek individualised support or referral to other services. There is also a follow-up with you 3 months after completion.  

This program is NOT available if you

  • have significant cognitive impairment that may impact upon participation 
  • have a significant mental illness that may impact on participation 
  • are currently having cancer treatment
  • have an infectious illness or are medically unstable 
  • are pregnant (please direct your query to Grampians Health Maternity Outpatient Services) 
  • have a Body Mass Index (BMI) lower than 18 
  • are over 75 years of age
