We care for adults (18+) with complex medical conditions who need support with their healthcare needs in the community or at home, without going to hospital.
You will be assigned a Health Coach and a Telehealth Caller who stay with you throughout your journey. This means you don't have to keep retelling your story, and we understand your health needs and goals.
Your Telehealth Callers (non-clinical) schedule your support bookings and have regular phone calls with you to check on your health. These calls help spot health declines quickly, support you at home, and avoid long hospital waits if issues can be resolved by your Health Coach, GP, or an after-hours service provider.
Your Health Coach is a clinical nurse who provides expert health information and advice about various medical conditions. They will listen to what's important to you and help you get the care you need at home.
By staying in touch, the Grampians Watch team will check on how you're feeling and support you if your health worsens. Both your Telehealth Caller and Health Coach provide ongoing support to promote confident independence at home. This includes:
