Geriatric Evaluation and Management Unit (GEM)

The Geriatric Evaluation and Management Unit provides assessment and rehabilitation of patients who have complex medical conditions.

Who we care for

We offer care to patients over aged 65 who:

  • have complex and multiple medical conditions
  • have functional and cognitive conditions requiring comprehensive assessment
  • need specialist assessment and management by multidisciplinary team
  • may have had multiple falls, dementia, delirium, stroke or fractures
  • may need to regain strength and function after a prolonged hospital stay

What you can expect

Our aim is to understand your functional capacity prior to admission, assess your current function, identify your goals and help you to achieve those goals by engaging you in activities and therapy.

The multidisciplinary team will involve you and family/carers, in all decision making and keep you all updated with your progress.

The team will recommend services required on discharge and support organising these services. Alternatively, a suitable care facility may be recommended.
