Disability Liaison Office

The Disability Liaison Officer (DLO) program helps people with disability get fair and safe healthcare.

Because people with disability know their own healthcare experiences best, the program listens to them. Disability Liaison Officers (DLOs) do not replace existing teams or services. They work with these teams to help ensure smooth and integrated healthcare.

Who we care for

DLOs help people with disabilities. They support people when they visit the hospital, go to the emergency department, have appointments, or stay in the hospital. The aim is to reduce the effects of being in the hospital.  

What you can expect

Our Disability Liaison Officers are based in Ballarat but can support all Grampians Health campuses. They can help you and your healthcare team through telehealth, phone, or in person.

How we can help

  • identify and fix issues that make it hard for you to access Grampians Health 
  • create a ‘Disability Care Plan’ with you which lists the supports you need so that others know how to help you and help put it into action
  • help your hospital care team understand your needs
  • be a contact point between families, health professionals, and support teams 
  • go with you to outpatient appointments to help you understand medical information
