Overview

The Diabetes Clinic provides clinical care for all diabetes types across the lifespan, from diagnosis to ongoing management.  

The diabetes multidisciplinary team includes endocrinology, paediatrics, general medicine, diabetes education, administration and allied health.  

Specialist services include: 

  • continuous glucose monitoring 
  • insulin pump initiation for paediatrics and adults 
  • bolus adviser meter service 
  • ambulatory insulin stabilisation 
  • new diagnosis Type 1 diabetes (adults and paediatrics) 
  • new diagnosis Type 2 diabetes (adults and paediatrics) 
  • gestational diabetes 

The Diabetes Centre at Ballarat is a member of the National Association of Diabetes Centres (NADC) and an accredited Tertiary Diabetes Service. We are a key provider of diabetes education to health professionals.

Access and referral process

Referrals are required from a GP or other health provider. These can be submitted via:

  • ARGUS / Health Link

  • In person drop off at the Diabetes Centre  
  • Bookings accepted for in person, Telehealth and telephone appointments

Referrals are required from a GP for endocrinologist or physician led clinics. Self-referrals are accepted to see the Diabetes Educator. These can be submitted via:
