Overview

The Cognitive Dementia and Memory Service (CDAMS) is a specialist diagnostic service for people experiencing memory loss or changes to their thinking, and for those who care for them. 

The CDAMS team offers expert clinical diagnosis, appropriate treatments, education, information about support services and direction in future planning. 

Referral process

Referrals should be sent to Central Intake

Referrals will be triaged. If accepted a letter will be sent to the client and the GP. If declined a letter will be sent to the GP. Copies of clinic letters will be faxed to the referrer/GP. 

Further referrals maybe made for a Neuropsychological Assessment, MRI, Occupational Therapy, Driving Assessments, ACAS, Dementia Australia and more. 

Referral criteria

Please include with your referral: 

  • Medical History Summary 
  • Current Medication List 
  • Hard copy of the Mini-Mental State Examination (MMSE) 
  • Clock Drawing Test (CDT) 
  • Full blood Examination (FBE) 
  • Urea and electrolytes (U&E) 
  • Liver function test (LFT) 
  • Calcium 
  • Thyroid function tests (TFT) 
  • Serum vitamin B12 and folate levels 
  • PO4 
  • HBA1C  
  • CT scan of brain without contrast 
  • ECG 

Investigations should all be within 6 months of the referral.
