About the program

You know yourself or the person you care for best. If you have a serious concern about your condition or notice a worrying change, REACH steps out the best way to let us know of your concerns.  

How does REACH work? 

  • You may RECOGNISE a worrying change or have a serious concern about your condition. Or you may recognise the worrying change in the person you care for. 
  • If you are worried, ENGAGE with those caring for you. Speak to the nurse, midwife, doctor, or allied health professional. Tell them your concerns. 
  • If they do not help with your concerns or the condition is getting worse, then 
    Ask to speak to the person in charge of the area. 
  • If you are still worried CALL 1800 298 626

    • Option 2. Horsham/Dimboola 
    • Option 3. Stawell 
    • Option 4. Edenhope 
      Your call will be answered by a staff member at Grampians Health. 

HELP is on the way. A health professional from Grampians Health will act on your concerns within 30 minutes. 

REACH

REACH posters are visible around our campuses. They are found in common areas, at the bedside, and in waiting rooms. These are designed to easily inform you of the clear pathway for you and your family to raise serious concerns when receiving care at Grampians Health.
