My Health Record
My Health Record is a secure online summary of your key health information. It allows your health information to be viewed securely online from anywhere, at any time – even if you move or travel interstate.
My Health Record content
My Health Record content can include:
- shared health summary
- test results
- hospital discharge summaries
- prescribed medications
- Medicare and PBS information
- immunisation history
You can also upload notes about yourself, such as information about allergies, current medications, and advance care plans.

Managing Your Record
You can set up access restrictions on your My Health Record if you wish.
Access at Grampians Health
Your treating clinicians may access your My Health Record (if you have one) via our Opal Digital Medical Record. Access is only permitted for the purpose of providing care to you, as required by law. If you have set up restricted access, you may be asked for your access code when you attend one of our sites.
We currently upload discharge summaries, pathology and radiology reports to My Health Record (this may vary depending on your Grampians Health campus). If you do not wish for us to contribute documents to your My Health Record, please advise Health Information Services on 03 5320 4227 or speak to your treating clinician.
